PHS is a community-based agency dedicated to providing consistent, respectful support for individuals and families through waiver-funded services in Pennsylvania. Our goal is simple: help people live fuller, more independent lives — at home and out in the community — while building trust through professionalism, communication, and follow-through.

We provide:

• In-Home & Community Supports — helping individuals develop skills, routines, and confidence at home and in the community

• Companion Supports — offering connection, encouragement, and dependable presence for social and everyday support

• Community Participation Supports — helping individuals engage in activities, build relationships, and participate meaningfully in community life

• Respite — providing support and supervision to ensure individuals are safe and cared for, while giving families and caregivers time to rest, recharge, or manage other responsibilities

Services are currently available in the following counties: Allegheny, Clearfield, and Jefferson
